Description
Large companies typically have an individual or group fully or partially dedicated to root cause failure analysis (RCFA). Medium and smaller-sized companies generally don't have enough resources to focus on RCFA, and the maintenance or operations manager is placed in charge. Because of their busy schedules, RCFA is often rushed or ignored. Consultant services might be the solution led by either the individual in charge or someone outside the organization.
